In single-player mode, the game becomes a real challenge. It’s an ultra-hard difficulty where getting caught means game over. So you’ll need to learn from your mistakes and progress by dying and trying again. You can’t carry the monsters easily like in multiplayer, so you’ll have to carefully move them while always being on the lookout for enemies. So you need to avoid being detected, avoid traps, and move carefully. In this mode offers a truly scary experience for players who want a serious horror challenge.

Your mission is to explore the hospital, find the «TEST-SUBJECT» monsters, and take them to the elevator. But watch out—there are traps and dangers everywhere. Some areas require teamwork to access, so it’s better not to go solo if you can avoid it. So you’ll find useful tools hidden in lockers, shelves and other places, including advanced equipment that might help you survive.

While you search you’ll likely encounter unknown enemies. They are drawn to noise, so stay quiet and hide when you can. If they spot you, run and throw items at them to buy time. If anyone gets caught, you can try to rescue them, but be quick—there’s only a short window to save them.

Once you find a «TEST-SUBJECT», you need to carry it to the elevator, but it makes a lot of noise, attracting enemies. If you’re playing with others, you can use the «Connected Carry» system to move the capsule faster. But if your teamwork falls apart, it could mean disaster for everyone.

Each «TEST-SUBJECT» has a different value, and your goal is to recover the most valuable ones. Once you’ve met the recovery target, you’ll move to the next floor by operating the elevator. But remember the deeper you go, the more dangerous it becomes.

In the game features random hospital floors that are sometimes completely different from what you’d expect. So you’ll face deadly nurse monsters and hunters that get stronger over time. You can exchange retrieval rewards for random items and carry useful tools with you to the next floor. But be warned—if you fail completely, you lose everything. How far can you go?

The game supports proximity-based voice chat, so you’ll hear your teammates up close.
